To The Parents/Guardians of LGLC Team Members:

Please complete the following to join the LGLC Booster Club. Advantages to booster
Membership include voting rights when applicable, one vote per individual parent or
guardian. There are no dues for the
2006 season.

In addition, the following Officer positions and Standing Committees are open to Booster
Club Members for enrollment and/or chairmanship. Chairs must approved by the President.

Please check next to any one(s) you may be interested in either chairing (note as such
please) or being an active member. You may check more than one of course. You are
not signing your life away…..we just need to know if you are interested in helping full or

As taken from the bylaws of the club:

Officers
The officers will consist of the President, Vice President, Secretary and Treasurer.
All Officers are automatically on the Board of Directors.

Duties

The following are guidelines for the duties for each Officer:

Joe Sexton-President – The President shall reside as the chief executive office of the
organization and shall preside over the meetings. The President shall have the authority
to assign duties to the other Officers as long as the duties are reasonably expected and
pertinent to each Officer’s level.

_________ - Vice President – Reports to the President - The Vice President shall replace the President temporarily
should the President need to be absent for meetings, and shall succeed to that position in the event that it becomes
vacant. He/she shall also be responsible for writing/revising the bylaws of the Club and presenting to the Board and
membership for approval. He/she shall also carry out any other duties that are reasonably expected for this level of office
as assigned by the President.

John Donnelly - Treasurer – Reports to the President - The Treasurer shall be the financial officer for the organization
and shall report an accounting of revenues and expenses at any organizational meeting and at the end and beginning of
each season. He/she shall assist the Sponsorship/donation and Fundraising committees in securing funds. In the event
the Club utilizes an accountant, the Treasurer will be the liaison between the Club and the accountant for all financial
matters. He/she shall also carry out any other financial duties as assigned by the President.

________- Secretary – Reports to the President - The secretary is responsible for creating and distributing agendas and
minutes for all meetings in a timely manner. He/she is also responsible for creating and updating roster information,
email and home addresses, phone numbers, and filing and updating any forms that are required by Team members,
parents/guardians, and Booster Club members. He/she shall also collect and keep tally of all reports produced by the
Standing Committees. He/she shall also carry out any other duties that are reasonably expected for this level of office as
assigned by the President.

All
Standing Committees are required to produce a duties list approved by the Board once a year and file a monthly
status report to the Secretary during the season and bimonthly during the off-season:

____ (Joe Sexton-Head Coach) Coaches Committee- Shall be chaired by the Head Coach - consists of all coaches,
works with scheduling committee, have exclusive authority for coaching styles, practice scheduling, game strategies,
discipline and team member playtime
____Team Manager Committee – Reports to Head Coach - Water, cups, towels for players, equipment management and
securing, assisting coaches and Field Committee as needed

____Scheduling Committee - arrange game and practice time scheduling, transportation for away games or observation
games, arranging for home game referees, field acquisition and time slots, works with coaches committee for scheduling
purposes

Kim Diehl (Chair) - Transportation Committee – assesses need for and arrange for transportation to practices, games and
social or educational events

____Field Committee – paint and refresh field lining, keeping supply of line paint and painter functional according to
specifications, tables, chairs, scorecard, clock, horn for home games, inspecting fields, assuring adequate cleanup

____Apparel Committee - research, order uniforms, practice jerseys, replace old, missing uniforms, manage laundry of
uniforms, and decide on logos, colors,

____Equipment Committee - in charge of obtaining and maintaining practice and game goals, balls, stick repair kits,
field lining equipment and paint, scorecards, clocks, horns, ensures all equipment within accepted rules and regulations
including player-owned equipment, arranges with retailers for discounts and demos

___Team Records Committee – collects and presents team statistics

____Fundraising Committee -research, schedule, organize, run, collect and report on all fundraisers

____Sponsorship/Donation Committee - Attract sponsors and donators, collects such monies and forward them to the
Secretary, administer receipts and gratitude letters.

____Marketing / Website Committee - advertise the Club, contact with press for game results and stories, recruit players,
coaches, organize and keep updated website, designs programs for games and works with Sponsorship/donation
committee for appropriate advertisements

____Risk Management/Insurance Committee - researching and obtaining adequate team and coach, Directors and
Officer's insurance, assessing risks for equipment, risks for legal issues

____Legal Counsel - reviewing and preparing any legal documents, advising on legal implications of various issues as
needed, advice on any nonprofit and tax-exempt statuses of the Club

____Audio Committee – arranges for video capture of games and other events as needed

____Social/Educational Committee – arranges for non-practice, game events for the team including field trips,
observation games, dinners, awards dinners

____Medical Committee - assessing /treating patient injuries, reviewing medical histories and patient risks/medical kit
acquisition, arrange to have Medical Release forms available at every practice/game and event related to LGLC.

____Feeder Program Committee – helps institute and encourage youth feeder programs and clinics through area middle
and elementary schools - and supports establishment of other area high school level Clubs, organizes clinics with
elementary and middle schools
